Uterus normal size. Both tubes and ovaries normal.
Procedure
Maylard incision of about 16 cm, 4 cm above the pubic symphysis. Inferior epigastric vessels ligated with 2-0 polyglactin 910 (Vicryl). Rectus muscles divided with cautery after U-sutures (2-0 delayed absorbable) fixed each muscle to the anterior sheath. Peritoneum opened transversely. Self-retaining retractor placed, bowel packed into the upper abdomen.
Round ligaments divided and broad ligaments opened on both sides. Paravesical and pararectal spaces developed and the cardinal ligaments felt between them. Operable.
Utero-ovarian ligaments clamped, cut and tied with polyglactin 910 (Vicryl) 1-0. Both tubes removed with the specimen, both ovaries kept.
Bilateral pelvic lymphadenectomy: common iliac, external iliac, internal iliac and obturator nodes removed on each side. Obturator nerves seen and preserved on both sides.
Ureters freed from the medial leaf of the broad ligament. Uterine arteries ligated at their origin from the internal iliac arteries. Superficial uterine veins ligated.
Bladder reflected off the cervix and upper vagina by sharp dissection.
Ureteric tunnels unroofed on both sides: roof of vesicouterine ligament clamped, cut and tied. Ureters freed from the cervix and rolled laterally, ureteric sheath intact. Part of the ventral parametrium above the ureter removed with the specimen; part below the ureter, carrying the bladder nerve branches, kept (C1).
Lateral parametria divided from the medial side of the internal iliac vein down to the deep uterine vein on both sides (C1). Tissue below the deep uterine vein left in place.
Pouch of Douglas peritoneum incised, rectovaginal space opened. Uterosacral ligaments divided at the level of the rectal attachment. Hypogastric nerve branches in the mesoureter seen and kept on both sides.
Paracolpos divided. Wertheim clamp placed on the vagina below the cervix. Vagina divided below the clamp, removing about 2 cm of vaginal cuff. Specimen removed en bloc.
Vaginal vault closed with continuous polyglactin 910 (Vicryl) 1-0.
Haemostasis checked on both side walls and at the vault. Both ureters seen along their pelvic course, intact.
Closure
Pelvic peritoneum not closed. No drain.
Fascia closed with running delayed absorbable polydioxanone (PDS) No. 1, bites 1.5 cm from the edge. Lateral extensions closed in the external oblique fascia only. Skin closed with staples.
Urine clear, about 250 ml during the operation.
Specimens and implants
Specimen 1: uterus with cervix, both parametria, 2 cm vaginal cuff and both tubes, stitch marking the anterior surface. Sent in 10% formalin for HPE.
Specimens 2 to 9: pelvic lymph nodes, right and left common iliac, external iliac, internal iliac and obturator, in separate labelled containers for HPE.
Frozen section: right obturator node, reactive, reported at 10:25.

Must document for this operation
- Type of radical hysterectomy done on each side (for example Querleu-Morrow B, C1 or C2). Words like 'Wertheim' or 'radical' mean different amounts of parametrium in different units, and late bladder and bowel problems depend on it.
- Para-aortic and pelvic node findings, with any frozen section results and times. They decide whether the operation should have continued and whether adjuvant chemoradiation is needed.
- State of the parametria and pelvic side walls after opening the paravesical and pararectal spaces. This is the operability decision, and it must be on record if the case is later questioned.
- What was done with the ovaries and whether this matched the consent. Removing ovaries without consent is an extra procedure that is hard to defend, and the gynaecologist needs to know about hormone status.
- Length of vaginal cuff removed and how the vault was closed. The pathologist reports the vaginal margin against this, and it explains later sexual symptoms.
- Ureters seen intact at the end, and any ureteric or bladder injury with its repair. Ureteric fistula and stricture are the most serious complications, and a later urologist needs to know what was found in theatre.
- Names of all anaesthetists and assistants, and who wrote the note. In a long cancer operation many people take part, and omitting names has gone against hospitals in negligence cases.
Common mistakes in these notes
- Writing 'Wertheim's hysterectomy done' with no type, no side-wise extent and no node groups.
- Putting all lymph nodes in one jar with no side or group label.
- Not recording the frozen section result that justified continuing the operation.
- Leaving out the catheter plan, so the bladder is allowed to overfill on the ward.
- Not writing that the ureters were seen intact at the end.
- Not noting ovarian transposition clips, which a radiation oncologist needs to find on X-ray.
Indian practice and paperwork
- Consent must explain the procedure, its risks, benefits, alternatives and who will do it, in a language the patient understands. The option of abandoning the operation and the plan for the ovaries should be part of it. (NABH Hospital Accreditation Standards 5th ed. (2020), PRE.4(c))
- An additional procedure without consent (for example removing ovaries that the patient wanted kept) is defensible only when it was needed to save life or health and could not wait. (Samira Kohli v. Dr Prabha Manchanda, (2008) 2 SCC 1)
- Name every anaesthetist in the note. A hospital was held negligent where the operation notes omitted one of two anaesthetists. (Thomas J. Indian J Urol 2009;25(3):384-8, PMID 19881136)
- Tissue not sent for HPE is yellow category (a) human anatomical waste. Discarded formalin from specimen jars is yellow (f) chemical waste. (Bio-Medical Waste Management Rules 2016, DGHS/CPCB guidelines)
- The operative note must be written before the patient is moved out of recovery, and post-op care must follow a written plan. (NABH 5th ed., COP.14(e) and (f))
