Worked example

Fictional case: Mrs. R.P., 34 years, P2L2 (both vaginal deliveries), something coming out per vaginam for 2 years. Family complete but wishes to keep her uterus. Uterovaginal prolapse with elongated cervix, cystocele and rectocele. Declined sterilisation (husband had vasectomy).

Case details

Date: 10/09/2026. Elective Fothergill's operation. OT 3.

In OT 09:00. Spinal 09:10. Incision 09:30. End 11:00.

Surgeon: Dr C. (consultant). Assistant: Dr A. (3rd-year resident, did D&C, anterior colporrhaphy and colpoperineorrhaphy under supervision). Second assistant: Dr B.

Anaesthetist: Dr D. Scrub nurse: Sr E.

Indication and pre-op

POP-Q: Aa +1, Ba +2, C +2, D -4, Ap 0, Bp +1, gh 4.5, pb 2.5, tvl 9. Stage III. Cervix healthy, no ulcer.

Cough stress test with prolapse reduced: negative. Pap smear 02/08/2026: negative. USG: uterus normal size, endometrium 7 mm, ovaries normal.

Consent: general, procedure-specific (including cervical stenosis, effect on future pregnancy and labour, recurrence) and anaesthesia consent in Marathi. Blood group AB positive, 1 unit cross-matched.

WHO checklist done. Inj cefazolin 2 g IV at 09:18.

Anaesthesia and position

Spinal anaesthesia. Dorsal lithotomy. Vulva and vagina painted with povidone-iodine and draped. Bladder emptied with metal catheter.

Findings

EUA: cervix reaches 2 cm beyond the introitus. Uterus anteverted, normal size. Uterine sound 10 cm (cervix about 5 cm). Midline cystocele. Rectocele in lower two-thirds with lax perineum. No enterocele.

Procedure

D&C: cervix dilated to Hegar 8. Scanty curettings, sent for HPE.

Anterior vaginal wall and cervicovaginal junction infiltrated with dilute vasopressin (20 units in 100 ml NS), 30 ml.

Inverted T incision on anterior vaginal wall, vertical limb to 1.5 cm below the urethral meatus. Flaps dissected. Vesicocervical ligament cut, bladder pushed up to the uterovesical peritoneum.

Transverse incision carried round the posterior cervicovaginal junction. Lateral and posterior vaginal walls separated from the cervix.

Mackenrodt's ligaments with descending cervical vessels clamped on each side at the level of amputation, cut and ligated with chromic catgut No. 1. Ends held long.

No enterocele found.

Cervix amputated 3 cm from the external os.

Posterior lip covered with the posterior vaginal flap by Sturmdorf suture with absorbable suture.

Mackenrodt's ligaments brought in front of the cervix with Fothergill's stitch on each side, drawing the cervix up and back. Anterior lip then covered.

Pubocervical fascia plicated with interrupted chromic catgut No. 0 (4 sutures). Excess anterior vaginal skin trimmed. Anterior wall closed with interrupted polyglactin 910 (Vicryl) 2-0.

Hegar 6 dilator passes easily through the new os.

Posterior colpoperineorrhaphy: flaps dissected, rectovaginal fascia plicated with interrupted Vicryl 2-0, finger in rectum. Levator ani and perineal body approximated with 3 interrupted chromic catgut No. 1 sutures. Posterior wall and perineal skin closed with Vicryl 2-0.

Closure

Vagina admits 2 fingers. Rectal examination: no sutures felt. Vaginal pack placed. Foley catheter inserted, urine clear.

Specimens and implants

Amputated cervix 3 cm and endometrial curettings sent for HPE in separate labelled containers in formalin.

End of procedure

Estimated blood loss 250 ml. No transfusion.

Mop, needle and instrument count correct.

No bladder or rectal injury.

Shifted to recovery, pulse 82/min, BP 116/74 mmHg.

Post-op orders

Pulse, BP and pad check every 30 min for 2 hours, then 4-hourly.

Vaginal pack to be removed after 6 hours. Foley catheter till pack is out, then remove and check voiding.

Oral fluids after 4 hours, then normal diet. Stool softener; avoid straining.

Early mobilisation, stockings.

No intercourse for 6 weeks. Report if periods do not come or come with severe pain. Tell obstetrician about this operation in any future pregnancy.

HPE review at follow-up.

Must document for this operation

  • Why the uterus was kept, and that the risks to future pregnancy were explained. Cervical amputation can cause cervical weakness and difficult labour, so a future obstetrician and any complaint review will look for this counselling.
  • Uterine sound length and the length of cervix removed. This shows the operation matched the cervical elongation and helps the pathologist and the next surgeon.
  • How the cardinal ligaments were fixed and how the stump was covered. These are the steps that give apical support; if prolapse recurs, the next surgeon needs to know what was done.
  • Patency of the new cervical os at the end. Cervical stenosis with haematometra is a known late complication, and the note shows the os was checked.
  • Curettings and cervix sent separately for HPE. An unexpected endometrial or cervical lesion must be traced to the right specimen.
  • Rectal examination and vaginal calibre at the end. These show rectal injury and over-narrowing were looked for.

Common mistakes in these notes

  • Writing 'Fothergill's done' without listing the five parts (D&C, amputation, ligament plication, anterior repair, posterior repair).
  • Not recording the uterine sound length or length of cervix amputated.
  • Sending cervix and curettings in one container.
  • Not writing that the os was checked for patency.
  • Not recording counselling about future pregnancy in a young woman.
