Worked example

Fictional case: Mr. S.M., 64 years, right eye neovascular glaucoma after central retinal vein occlusion. Right eye perception of light only, painful, IOP 46 mmHg on four glaucoma drugs.

Case details

Date: 22/05/2026. Elective. Eye OT 2.

Block 11:10. Start 11:25. End 11:40.

Surgeon: Dr A. (2nd-year resident, Ophthalmology). Consultant: Dr C., present in OT. Scrub nurse: Sr E.

Indication and pre-op

Right eye: PL present, PR inaccurate. Rubeosis iridis, closed angle on gonioscopy. IOP 46 mmHg by applanation at 09:30 on timolol 0.5%, brimonidine, dorzolamide and oral acetazolamide. Severe pain and redness.

Left eye 6/9, IOP 16 mmHg.

Indication: painful eye with no useful vision and high IOP on full medical treatment. Low chance of success with filtering surgery in neovascular glaucoma.

Consent: general, procedure-specific and anaesthesia consent from patient in Telugu. Explained that the aim is pain relief and pressure control, not vision, and that repeat treatment may be needed.

Right eye marked. WHO checklist sign in and time out done. No systemic antibiotic.

Anaesthesia and position

Topical proparacaine 0.5%. Peribulbar block, right eye, 6 ml of 2% lignocaine and 0.5% bupivacaine (2:1) with hyaluronidase, given by Dr A. Ocular compression 10 minutes. Supine. Lids and periocular skin painted with povidone-iodine and draped. Wire speculum.

Findings

Cornea hazy with epithelial oedema. AC normal depth, no hyphaema. Rubeosis at the pupil margin. No scleral thinning.

Procedure

Cyclocryotherapy, right eye, with the 4 mm cyclocryo probe.

Probe placed just behind the limbus. Each freeze at -80 C held for 60 seconds, ice-ball seen, allowed to thaw on its own before lifting the probe.

Three applications in each of the two inferior quadrants (from 3 to 9 o'clock through 6 o'clock), 6 applications in all. Superior 180 degrees left untreated.

Prednisolone acetate 1% drop instilled at the end.

Closure

Speculum removed. Pad and bandage applied.

End of procedure

Blood loss nil. No complications.

Instrument count correct.

Specimens: none.

Shifted to ward. Pulse 78/min, BP 138/84 mmHg.

Post-op orders

Check right IOP 1 hour after the procedure. Inform resident if IOP is higher than before treatment or pain is severe.

Mild oral analgesic as charted, as needed for pain.

Prednisolone acetate 1% eye drops four times a day for 10 days, then taper as per inflammation.

Continue timolol, brimonidine, dorzolamide and acetazolamide for now; stop one by one as IOP allows.

Review tomorrow with IOP. Repeat treatment, if needed, not before 3 weeks.

Must document for this operation

  • Quadrants and clock hours treated, and those left untreated. Any repeat treatment is planned around the untreated area, and treating all 360 degrees raises the risk of hypotony and phthisis.
  • Energy settings: temperature and freeze time for cryo, or power, duration and final power for diode. The dose explains the outcome and is the starting point if the treatment has to be repeated.
  • Number of applications. Needed to compare cumulative treatment across sessions.
  • Visual acuity of both eyes and IOP (intraocular pressure) before treatment. Loss of vision is a known risk, and the record shows what vision the eye had before.
  • The patient was told that the aim is pain and pressure control, not better vision. Expectations are the main source of complaints after cyclodestruction.
  • Previous cyclodestructive sessions and their dates. The cumulative area treated decides whether it is safe to treat again.

Common mistakes in these notes

  • Writing 'cyclocryo done' with no quadrants or number of applications.
  • Not recording the diode power at which pops were heard or the final power used.
  • No IOP check ordered after treatment.
  • Not writing that the fellow eye was examined.
  • Stopping all glaucoma drugs straight after treatment without a written plan.

Indian practice and paperwork

  • NABH expects the operative note to be written before the patient is moved out of recovery, and post-op care to follow a written plan. For cyclodestruction the plan should say which glaucoma drugs continue. (NABH 5th ed., COP.14(e) and (f))
  • Consent must cover the procedure, risks, benefits and alternatives in a language the patient understands, taken by the person doing the procedure. (NABH 5th ed., PRE.4(c) and (e))
