Worked example
Fictional case: Ms. S.K., 22 years, painless swelling of the left upper lid for 2 months, not settled with hot fomentation and antibiotic drops. First episode.
Case details
Date: 04/05/2026. Elective, minor OT. Start 11:10, end 11:25.
Surgeon: Dr A. (1st-year resident, Ophthalmology). Assistant: Sr B. Consultant: Dr C., available in the department.
Indication and pre-op
Left upper lid chalazion, central third, about 6 x 5 mm, pointing on the conjunctival side, 7 mm from the lash line. Single. No previous I&C.
Mild posterior blepharitis both eyes. VA 6/6 both eyes.
Consent: procedure-specific consent taken in Marathi, including residual swelling for 2 to 3 weeks and recurrence. Left eye marked.
Time out done: patient, left upper lid, procedure confirmed.
Anaesthesia and position
Supine. Amethocaine 1% drops, 4 drops in the left conjunctival sac. Lignocaine 2% with adrenaline 1:100000, 1 ml infiltrated at the upper tarsal border and on either side of the lesion.
Lid and surrounding skin painted with povidone-iodine and draped.
Procedure
Chalazion clamp applied, ring on the palpebral conjunctiva with the lesion in its centre. Lid everted.
Vertical incision about 5 mm on the conjunctival side over the lesion with a No. 11 blade, along the line of the gland, blade moving away from the eye.
Findings: gelatinous granulation tissue, confined to the tarsus, one small loculus in the lateral part. Thin wall. Nothing firm or suspicious.
Contents removed with a small curette. Loculus opened and curetted. Cavity swabbed dry. Wall not excised. No cautery used.
Clamp released. Pressure for 1 minute. Bleeding stopped.
Closure
No sutures. Antibiotic eye ointment (chloramphenicol) instilled. Pad applied for 1 hour.
End of procedure
Blood loss negligible. No complications.
Specimen: not sent for HPE (first episode, young patient, typical contents).
Patient walked out of the minor OT, comfortable.
Post-op orders
Remove pad after 1 hour.
Antibiotic eye ointment (chloramphenicol) to left eye three times a day for about a week.
Hot fomentation twice a day from tomorrow. Oral analgesic if needed for pain.
Told that a small lump may persist for 2 to 3 weeks. Lid hygiene for blepharitis.
Review in the eye OPD after 1 week, earlier if pain, spreading redness or blurred vision.

Must document for this operation
- Eye, upper or lower lid, and position along the lid. Lid surgery is small and easy to do on the wrong side, and the next doctor needs to know which lesion was treated if another appears.
- Whether the chalazion was recurrent at the same site. A recurrent lesion at the same site should be sent for histopathology (HPE) to rule out sebaceous gland carcinoma.
- What the contents looked like. Firm tissue with no gelatinous material suggests a tumour, and the note shows why tissue was or was not sent.
- Side of the incision (conjunctival or skin) and its direction. A skin incision leaves a scar that the patient will ask about, and a wrong direction on the conjunctival side can damage other glands.
- Whether loculi were looked for and cleared. Leftover pockets of granulation tissue are a common reason for early recurrence.
- Specimen sent or not, with the reason. If a later biopsy shows carcinoma, the first note will be read to see whether tissue was examined.
Common mistakes in these notes
- Writing 'chalazion I&C done' with no eye, lid or site.
- Not recording that the chalazion was recurrent, then not sending the tissue.
- Leaving out the local anaesthetic drug, strength and volume.
- Not writing the appearance of the contents.
- Forgetting to tell the patient, and note, that some swelling may stay for 2 to 3 weeks.
Indian practice and paperwork
- Consent must explain the procedure, risks, alternatives and who will do it, in a language the patient understands, and be taken by the person doing the procedure. (NABH 5th ed., PRE.4(c) and (e))
- Tissue not sent for HPE is human anatomical waste and goes in the yellow bag. Discarded formalin is yellow (f) chemical waste. (Bio-Medical Waste Management Rules 2016, CPCB guidelines)
- Even for a minor OT case, the operative note should be written before the patient leaves the recovery area. (NABH 5th ed., COP.14(e))
