Worked example
Fictional case: Mrs. F.B., 35 years, 60 kg, dull right upper quadrant pain for 6 months, USG and CT show a single 10 cm hydatid cyst (WHO CE2, multivesicular with daughter cysts) in segments 5 and 6 reaching the liver surface, LFT normal, CBD not dilated, no lung cyst, on albendazole for 4 weeks.
Case details
Date: 12/01/2027. Elective. OT 1.
In OT 09:00. Induction 09:15. Incision 09:35. Closure completed 12:05.
Surgeon: Dr C. (Consultant, General Surgery). Assistant: Dr A. (3rd-year resident), who opened, helped with evacuation and closed. Dr B. (1st-year resident) observing.
Anaesthetist: Dr D. Scrub nurse: Sr E.
Indication and pre-op
Indication: symptomatic 10 cm multivesicular hydatid cyst, segments 5 and 6. PAIR not suitable (multivesicular with daughter cysts).
Albendazole 400 mg twice a day with meals (60 kg: about 13 mg/kg/day, inside the 10 to 14 mg/kg/day range) for 4 weeks before surgery.
Consent: open evacuation of hydatid cyst with partial pericystectomy and drainage, possible T-tube or liver resection. Risks of allergic reaction, spill and recurrence, bile leak and bleeding explained in Kashmiri and Hindi.
2 units cross-matched. Anaesthetist informed of anaphylaxis risk, drugs ready. WHO checklist done. Inj cefazolin 2 g IV at 09:20, before incision.
Anaesthesia and position
General anaesthesia with endotracheal intubation. Supine. Nasogastric tube and Foley catheter in situ. Parts painted with povidone-iodine and draped.
Procedure
Right subcostal incision. Abdomen explored: no cysts in peritoneum, omentum or spleen.
Findings: single 10 cm cyst bulging on the inferior surface of segments 5 and 6, flimsy adhesions to omentum and hepatic flexure. Pericyst thin, not calcified. Gallbladder normal.
Adhesions separated and liver mobilised. Area around the cyst packed with dark mops soaked in 20% hypertonic saline.
Cyst punctured through the dome and aspirated with a closed suction system: 300 ml of clear fluid, not bile-stained. 20% hypertonic saline injected into the cyst and left for 15 minutes.
Stay sutures on the cyst wall. Cyst opened for 5 cm. About 600 ml of contents with many daughter cysts and the laminated membrane removed with sump suction, sponge-holding forceps and a spoon.
Cavity wiped with hypertonic saline soaked gauze and packed with 20% hypertonic saline soaked gauze for 10 minutes.
White mop placed in the cavity: one small spot of bile staining on the posterior wall. 2 mm biliary opening found and closed with polyglactin 910 (Vicryl) 4-0. White mop test repeated: no bile.
Redundant pericyst roof excised (partial pericystectomy). Cut edge oversewn with continuous absorbable suture for haemostasis and small ducts.
Omental pedicle placed into the residual cavity and fixed to the rim with absorbable sutures (omentoplasty).
Scolicidal mops removed carefully. Peritoneal cavity washed with warm saline. No spill seen.
Closure
One tube drain beside the omental plug in the cavity and one in the right subhepatic space, both through separate stab wounds, fixed and labelled (1 cavity, 2 subhepatic).
Abdominal wall closed in layers with continuous looped polydioxanone (PDS) No. 1. Skin with staples.
Specimens and implants
Laminated membrane, daughter cysts and excised pericyst sent for HPE in formalin. No implants.
End of procedure
Estimated blood loss 250 ml. No transfusion.
Mop, needle and instrument count correct (including coloured mops), confirmed by scrub nurse.
No anaphylaxis. No intra-operative complications.
Extubated in OT. Shifted to surgical HDU. Pulse 90/min, BP 118/74 mmHg.
Post-op orders
Pulse, BP, temperature and urine output hourly for 6 hours. Inform at once if rash, wheeze or BP falls.
Chart cavity and subhepatic drains separately. Inform if bile in either.
Serum sodium in the evening and on POD 1.
Nil by mouth, nasogastric tube on free drainage. IV fluids as charted.
Inj paracetamol 1 g IV 8-hourly. Incentive spirometry.
Restart albendazole 400 mg twice a day with meals when taking orally, to continue for 1 month. USG of the liver at follow-up.

Must document for this operation
- Number, site, size and type of cyst, and why the chosen operation was used. Recurrence and residual cavities are judged against this at follow-up imaging.
- Scolicidal agent, its strength and the contact time. Scolicidal agents can cause sclerosing cholangitis and high sodium, the note shows which agent was used and why.
- Whether the fluid was bile-stained and how biliary communication was looked for and managed. Bile leak is the most important complication, and the next team needs to know whether a fistula was found and closed.
- Any spill of cyst contents and what was done. Spill raises the risk of anaphylaxis and of peritoneal recurrence and changes follow-up.
- Method used for the residual cavity and position of each drain. Post-op collections and drain outputs are interpreted against this.
- Albendazole before and after surgery. Drug cover lowers recurrence risk, and the post-op plan must continue it.
Common mistakes in these notes
- Writing 'hydatid cyst removed' without the operation type or cavity management.
- Not naming the scolicidal agent or its strength and time.
- No mention of how biliary communication was checked.
- Not counting the coloured mops separately.
- No albendazole plan in the post-op orders.
Indian practice and paperwork
- Consent should name the likely extra procedures (T-tube, liver resection). A common consent for foreseen additional steps is valid, an unconsented extra procedure is defensible only when needed to save life or health. (Samira Kohli v. Dr Prabha Manchanda, (2008) 2 SCC 1)
- Laminated membrane and pericyst not sent for HPE are human anatomical waste for the yellow bag. Discarded formalin is chemical waste, yellow (f). (Bio-Medical Waste Management Rules 2016, CPCB guidelines)
- Mops, needles and instruments must be counted before the start, before final closure and at the end. Record the count. (NQAS Assessor's Guidebook for District Hospitals 2020, ME E15.3)
