Worked example

Fictional case: Mr. P.D., 22 years, bilateral breast enlargement for 4 years, firm disc of tissue under both areolae, Simon grade 2a, no drug cause found, testes normal, ultrasound shows bilateral subareolar glandular tissue with no suspicious lesion.

Case details

Date: 03/09/2026. Elective. OT 4.

In OT 09:10. Induction 09:20. Incision 09:40. Closure completed 11:05.

Surgeon: Dr A. (3rd-year resident, General Surgery). Assistant: Dr B. (1st-year resident). Consultant: Dr C., present in OT.

Anaesthetist: Dr D. Scrub nurse: Sr E.

Indication and pre-op

Indication: bilateral glandular gynaecomastia, Simon 2a, present for more than 1 year, causing distress. No drug cause, liver and kidney function normal.

Marked standing on the ward: midline, sternal notch, both NACs, inframammary folds, lateral border of pectoralis major, area of excess tissue. Clinical photographs taken with consent.

Consent: bilateral excision through periareolar incisions. Risks of haematoma, seroma, saucer deformity, nipple numbness and asymmetry explained in Marathi.

WHO checklist done. No antibiotic prophylaxis (clean procedure, no risk factors).

Anaesthesia and position

General anaesthesia. Supine, arms abducted on arm boards. Parts painted with chlorhexidine-alcohol and draped.

Procedure

Right side first. Inferior hemi-periareolar (Webster) incision along the edge of the areola from 3 to 9 o'clock.

Areolar flap raised in a deep plane, keeping a layer of tissue on the undersurface of the areola.

Findings: firm white disc of gland about 6 x 5 cm under the NAC, no discrete lump. Little fatty excess.

Gland dissected from the skin flaps with scissors and cautery and lifted off the pectoral fascia, keeping the fat over the fascia. A thin layer of tissue left under the NAC to prevent a saucer deformity.

Gland removed, 68 g. Edges feathered to a smooth contour.

Left side done in the same way. Gland 61 g.

Haemostasis with cautery. Both cavities washed with saline. Chest contour and NAC position symmetrical when checked from the foot end.

Closure

Suction drain on each side, brought out through a stab in the lateral chest wall.

Subcutaneous layer closed with polyglactin 910 (Vicryl) 3-0. Skin closed with subcuticular poliglecaprone (Monocryl) 3-0.

Compression dressing and compression garment applied. Both NACs pink at end.

Specimens and implants

Right breast tissue 68 g and left breast tissue 61 g, sent for HPE in separate labelled formalin jars. No implants.

End of procedure

Estimated blood loss 100 ml. No transfusion.

Mop, needle and instrument count correct, confirmed by scrub nurse.

No intra-operative complications.

Shifted to recovery awake and stable. Pulse 80/min, BP 124/78 mmHg.

Post-op orders

Pulse, BP and chest swelling every 30 minutes for 4 hours. Inform if either side becomes tense or a drain fills with blood.

Chart each drain separately. Remove when less than 20 ml in 24 hours on two consecutive days.

Compression garment day and night for 3 weeks except for bathing, then during the day to complete 6 weeks.

Tab paracetamol 500 mg, 2 tablets 6-hourly when needed, not more than 8 tablets in 24 hours.

Oral fluids when awake. Walk the same evening. Shower from POD 1 with the garment off only for the bath.

Review in OPD at 2 weeks with HPE report.

Must document for this operation

  • Grade of gynaecomastia and the cause looked for, including drugs and testes. Operating without excluding a treatable or dangerous cause (such as a testicular tumour) is hard to defend.
  • Weight of tissue removed from each side. Asymmetry complaints are judged against the weights, and the pathologist needs side-labelled specimens.
  • Thickness of tissue left under the nipple and areola. A saucer deformity is the commonest complaint after this operation, and the note shows it was guarded against.
  • Colour of the nipple and areola at the end of surgery. If necrosis appears later, the note shows the blood supply was checked in theatre.
  • Liposuction volume from each side, if used. It explains contour and fluid shifts and is needed if a revision is planned.
  • Standing markings and photographs taken before surgery. These are the reference for any later cosmetic dispute.

Common mistakes in these notes

  • Sending both sides in one jar, so a lesion cannot be lateralised.
  • Not recording the grade or the work-up for a cause.
  • Leaving out the weight of tissue removed from each side.
  • No note of nipple-areola colour at the end.
  • Not writing how long the compression garment is to be worn.

Indian practice and paperwork

  • Consent must explain risks, benefits and alternatives (including no surgery and liposuction alone) in a language the patient understands. (NABH 5th ed., PRE.4(c))
  • Breast tissue not sent for HPE is human anatomical waste for the yellow bag. Discarded formalin is chemical waste, yellow (f). (Bio-Medical Waste Management Rules 2016, CPCB guidelines)
