Worked example

Fictional case: Mrs. V.R., 28 years, G4P0A3 at 13+2 weeks, with three previous painless second-trimester losses at 18, 20 and 21 weeks, booked for history-indicated cerclage.

Case details

Date: 14/02/2026. Elective McDonald cervical cerclage. OT 4.

In OT 09:30. Spinal 09:40. Procedure started 09:50, ended 10:10.

Surgeon: Dr A. (3rd-year resident, OBGY). Assistant: Dr B. Consultant Dr C. present.

Anaesthetist: Dr D. Scrub nurse: Sr E.

Indication and pre-op

Indication: history-indicated cerclage. Three previous second-trimester losses with painless cervical dilatation (18, 20 and 21 weeks).

Scan on 12/02/2026: single live fetus, CRL matching 13 weeks, NT normal, placenta posterior upper.

No bleeding, no pain, no leaking. Speculum: os closed, discharge normal.

Consent explained in Hindi: risks, and removal planned at 36 to 37 weeks or earlier if labour or leaking. WHO checklist done. No antibiotic given as per unit plan for history-indicated cerclage.

Anaesthesia and position

Spinal anaesthesia. Dorsal lithotomy. Vagina gently cleaned with povidone-iodine and draped. Bladder emptied with a plain catheter.

Weighted posterior speculum and right-angle anterior retractor. Anterior and posterior lips of cervix held with sponge-holding forceps.

Findings

Cervix 2.5 cm long vaginal portion, firm, os closed. Membranes not visible. Discharge normal.

Procedure

McDonald cerclage with Mersilene tape on a round-bodied needle.

Purse-string started anteriorly just below the bladder reflection. Five bites taken round the cervix, deep into the stroma but not into the canal. Bites kept away from the uterine vessels at 3 and 9 o'clock. Posterior bite placed high.

Knot tied anteriorly with successive throws, internal os closed and fingertip admitted at external os. Ends cut 3 cm long for removal.

One stitch placed.

Closure

Minimal bleeding from needle points, stopped with pressure. No pack. Membranes not seen, no leak.

Fetal heart seen on ultrasound after procedure, 158 bpm.

Specimens and implants

Implant: Mersilene tape, make and lot number sticker pasted in file. No specimen.

End of procedure

Estimated blood loss 10 ml.

Swab and needle count correct, confirmed by scrub nurse.

No intra-operative complications.

Shifted to ward in stable condition. Pulse 80/min, BP 110/70 mmHg.

Post-op orders

Pulse, BP and temperature 4-hourly. Inform if pain, bleeding, leaking or fever.

Oral paracetamol as needed, dose as per unit protocol.

Discharge tomorrow if well. No bed rest needed.

TVS for cervical length at 18 weeks.

Cerclage removal planned at 36 to 37 weeks in OPD; come earlier with pain, bleeding or leaking. Written on antenatal card.

Must document for this operation

  • The indication type (history, ultrasound or examination) with the evidence: past losses with weeks, or cervical length in mm. Cerclage helps only in defined groups, and the indication is checked in audit and when deciding on cerclage in the next pregnancy.
  • Fetal viability and scan findings before the stitch. A lethal anomaly or non-viable pregnancy is a contraindication and must be excluded first.
  • Technique, suture material, number of bites and where the knot lies. The person removing the stitch must know what to look for, especially if it is a Shirodkar with a buried knot.
  • Length of the cut ends. Long enough ends allow removal in OPD without anaesthesia.
  • State of the membranes and os at the end. If she leaks or bleeds in the next days, this is the baseline for deciding whether the stitch caused it.
  • The removal plan and date. A stitch left in during labour can tear the cervix, so the plan must be clear to every doctor who sees her.

Common mistakes in these notes

  • Writing 'cerclage done' without McDonald or Shirodkar.
  • Not recording the suture material and where the knot is.
  • Not writing the cervical length or past history that justified the stitch.
  • Leaving out the fetal heart check after the procedure.
  • Not writing the removal plan on the antenatal card.

Indian practice and paperwork

  • The suture left in the cervix is an implanted material. NABH asks for the batch and serial number of implants to be recorded in the medical record, so paste the suture sticker or write the lot number. (NABH 5th ed. (2020) MOM.10(d))
