Worked example

Fictional case: Mrs. P.D., 46 years, with a 2 cm invasive ductal carcinoma in the left breast upper outer quadrant, clinically and sonologically node negative, planned for breast-conserving surgery with sentinel node biopsy.

Case details

Date: 08/04/2026. Elective. OT 4.

In OT 09:10. Induction 09:20. Skin incision 09:40. Closure completed 11:05.

Surgeon: Dr A. (3rd-year resident, General Surgery). Assistant: Dr B. (1st-year resident). Consultant: Dr C., present and scrubbed.

Anaesthetist: Dr D. Scrub nurse: Sr E.

Indication and pre-op

Diagnosis: carcinoma left breast, cT2 N0. Core biopsy: invasive ductal carcinoma grade 2, ER positive, PR positive, HER2 negative. Ultrasound size 2.1 cm, 2 o'clock, 5 cm from nipple. Axillary ultrasound normal.

Tumour and side marked on the skin by the operating surgeon in the ward with the patient awake.

Mapping: methylene blue dye planned. No history of dye or drug allergy.

Consent: general, procedure-specific and anaesthesia consent taken in Marathi from the patient, husband as witness. Consent includes re-excision, mastectomy if margins cannot be cleared, and axillary dissection if the node is positive.

Frozen section booked for sentinel node and margins.

WHO checklist sign in and time out done. Inj cefazolin 2 g IV at 09:22, single dose.

Anaesthesia and position

General anaesthesia with endotracheal tube. Anaesthetist informed that blue dye can lower the SpO2 reading.

Supine, left arm abducted to 80 degrees on an arm board. Left breast, axilla and arm painted with chlorhexidine-alcohol and draped.

Procedure

Methylene blue, diluted, 4 ml injected into the subareolar breast tissue (not intradermal) at 09:30. Breast massaged for 5 minutes.

Transverse incision of 4 cm in the lower axilla, below the hair-bearing skin. Axillary fascia opened. A blue lymphatic traced to two blue nodes at level I, lateral to pectoralis minor. Both excised after clipping the hilar vessels. No other palpable node. Nodes 1.2 cm and 0.8 cm, sent separately for frozen section at 09:55.

Curvilinear incision of 6 cm over the tumour at 2 o'clock. Skin flaps not raised widely.

Tumour 2 x 2 cm, hard, free from skin and pectoral fascia. Excised with about 1 cm of normal breast tissue on all sides down to the pectoral fascia. Fascia left intact.

Specimen oriented before removal: short suture superior, long suture lateral, clip on deep surface. Specimen inked as per pathology protocol and sent for frozen section of margins at 10:20.

Frozen section report at 10:45: both sentinel nodes free of tumour. All margins free, closest the superior margin at 4 mm. No re-excision needed.

Four medium haemoclips placed on medial, lateral, superior and inferior walls of the cavity for radiotherapy planning.

Haemostasis with bipolar cautery. Cavity walls checked with Allis forceps. Surrounding breast tissue mobilised off the pectoral fascia and approximated with interrupted polyglactin 910 (Vicryl) 2-0.

Closure

Axilla: subcutaneous layer interrupted Vicryl 3-0, skin subcuticular absorbable 3-0.

Breast: subcutaneous layer interrupted Vicryl 3-0, skin subcuticular absorbable 3-0. No drain.

Light gauze dressing and supportive brassiere.

Specimens and implants

1. Wide local excision specimen, left breast, oriented (short suture superior, long suture lateral, clip deep), for HPE and ER/PR/HER2.

2. Sentinel node 1, blue, left axilla, for HPE after frozen section.

3. Sentinel node 2, blue, left axilla, for HPE after frozen section.

Implants: 4 medium haemoclips in the cavity walls.

End of procedure

Estimated blood loss 80 ml. No transfusion.

Mop, needle and instrument count correct, confirmed by scrub nurse.

No intra-operative complications.

Extubated and shifted to recovery in stable condition. Pulse 82/min, BP 124/78 mmHg, SpO2 97% on room air.

Post-op orders

Pulse, BP and wound check every 30 min for 2 hours, then 4-hourly. Inform if the breast or axilla swells.

Oral liquids after 4 hours if no nausea, then normal diet.

Tab paracetamol as per drug chart.

Gentle movement of the left arm allowed from today. Supportive brassiere day and night.

Patient told that blue urine and blue skin are expected and will fade.

Planned discharge on POD 1. Review with final HPE in the breast clinic; tumour board and radiotherapy referral.

Must document for this operation

  • Orientation code of the breast specimen (which suture or clip marks which surface). The pathologist can only report which margin is involved if the orientation is written down, and the surgeon needs this to re-excise the correct wall.
  • Mapping agent, dose, site and time of injection. Failed mapping or a dye reaction is explained only by this record, and the anaesthetist needs it for a falling oximeter reading.
  • Number of sentinel nodes, and whether each was blue, hot or palpable. The pathology report must match the number removed, and a missing node is found only if the count is written.
  • Frozen section results and what was done with them. It explains why axillary dissection was or was not done in the same sitting.
  • Any cavity shaves or re-excised margins, labelled as new margins. A shave changes which surface is the final margin for the pathologist and radiotherapist.
  • Cavity clips: number and position. The radiation oncologist uses them to plan the boost, and they appear on later mammograms.
  • Side, quadrant and clock position of the tumour. Wrong-side and wrong-site errors in breast surgery are prevented and audited using this record.

Common mistakes in these notes

  • Writing 'lumpectomy done' with no margin distance or orientation code.
  • Sending all sentinel nodes in one container without saying how many were removed.
  • Not recording the mapping agent and time, so a failed mapping cannot be explained.
  • Forgetting to record cavity clips, so the radiotherapist does not know they are there.
  • Leaving out the frozen section time and result from the note.

Indian practice and paperwork

  • NABH standard COP.14(e) (5th edition) requires the operative note to be written before the patient is transferred out of recovery. (NABH Hospital Accreditation Standards 5th ed. (April 2020), COP.14 OE e)
  • In NQAS-assessed public hospitals, the OT checkpoint 'Operative Notes are Recorded' looks for the people present, pre- and post-op diagnosis, procedure, length of procedure, estimated blood loss, fluids given, specimen removed and complications. (NQAS Assessor's Guidebook for District Hospitals 2020, Vol I, ME E8.4)
  • The NHSRC SaQushal patient-safety tool checks that prophylactic antibiotics were given within 60 minutes before incision, so write the time given. (SaQushal Patient Safety Self-Assessment Tool (NHSRC/MoHFW, 2022))
