Worked example
Fictional case: Mr. P.R., 24 years, 4 cm soft painless swelling at the junction of the upper and middle third of the left sternomastoid for 8 months, one episode of infection 3 months ago settled with antibiotics.
Case details
Date: 14/05/2026. Elective. OT 2.
In OT 10:10. Induction 10:20. Skin incision 10:36. Closure completed 11:45.
Surgeon: Dr A. (3rd-year resident, General Surgery). Assistant: Dr B. (2nd-year resident). Consultant: Dr C., present in OT.
Anaesthetist: Dr D. Scrub nurse: Sr E.
Indication and pre-op
Indication: left second branchial cyst, previously infected, now quiet for 3 months.
USG and CECT neck: 4.2 x 3.1 cm thin-walled cyst deep to anterior border of left sternomastoid, lying on the carotid sheath, no solid part. FNAC: keratinous debris and squamous cells, no malignant cells.
Consent: general, procedure-specific and anaesthesia consent taken in Marathi. Risk to lower lip and tongue nerves, haematoma and recurrence explained.
WHO checklist sign in and time out done. No antibiotic prophylaxis (clean neck procedure, cyst not infected now).
Anaesthesia and position
GA with orotracheal tube led out at the right corner of the mouth. Supine, sandbag under shoulders, head ring, face turned to right.
Parts painted with povidone-iodine and draped.
Procedure
7 cm transverse incision in a skin crease over the swelling. Platysma divided. Upper flap raised only enough for access. External jugular vein preserved. Greater auricular nerve seen and preserved.
Investing fascia opened at the anterior border of the sternomastoid and the muscle retracted backwards.
Findings: 4 cm smooth cyst, thin wall, mildly adherent to the internal jugular vein after previous infection. No tract above the cyst.
Cyst freed close to its wall from the facial vein, then from the internal jugular vein and carotid sheath. Superiorly, posterior belly of digastric retracted and hypoglossal nerve seen and preserved. Two pharyngeal veins ligated with polyglactin 910 (Vicryl) 3-0.
Cyst removed intact.
Haemostasis with bipolar cautery. Wound washed with saline.
Closure
10 Fr suction drain placed through a separate stab posterior to the incision.
Platysma closed with interrupted Vicryl 3-0. Skin closed with interrupted polyamide (Ethilon) 4-0. Dry dressing.
Specimens
Intact left branchial cyst sent for HPE in formalin.
End of procedure
Estimated blood loss 50 ml. No transfusion.
Mop, needle and instrument count correct, confirmed by scrub nurse.
No intra-operative complications. Lower lip symmetrical and tongue protrudes in midline after extubation.
Shifted to recovery in stable condition. Pulse 80/min, BP 122/78 mmHg, SpO2 99% on room air.
Post-op orders
Watch neck for swelling under the wound and for difficulty breathing. Inform surgeon at once if neck swells.
Drain output charted 8-hourly.
Sips when fully awake, then normal diet.
Tab paracetamol 1 g 6-hourly.
Check lower lip, tongue and shoulder shrug on morning round and record.
Review in clinic at 10 days with HPE report; skin sutures removed at that visit if the wound has healed.

Must document for this operation
- Side, and whether the lesion was a cyst, sinus or fistula. Wrong-side surgery and a mismatch with the consent are both easy to prove from the note.
- The upper limit of the tract and how it ended (blind, ligated, or into the pharynx). If a sinus recurs, the next surgeon needs to know how much of the tract was taken.
- Nerves seen and preserved, especially the hypoglossal and marginal mandibular. If tongue or lip weakness appears later, the note shows whether the nerve was seen intact.
- Whether the pharynx was opened and how it was closed. The ward team must know this to plan feeding and antibiotics.
- Whether the specimen was removed intact or ruptured. Spillage or a piece left behind raises the chance of recurrence and should be on record.
- Pre-operative FNAC (fine needle aspiration cytology) or imaging that excluded a malignant node. A cystic metastasis can look like a branchial cyst in adults, and the note must show this was considered.
Common mistakes in these notes
- Not writing the side of the neck in the procedure line.
- Writing 'cyst excised' for a sinus without describing the tract and its upper end.
- Leaving out whether a second (stepladder) incision was made.
- No record of nerve function after extubation.
- Not stating why the drain was placed and where it exits.
